Social Determinants of Health Grants, $25,000 – $250,000
The Social Determinants of Health grants focus on the factors that impact health outcomes. They are the conditions in which people are born, grow, live, and work and the systems shaping the conditions of people’s daily lives. Grants will be awarded that address the following categories:
- Economic stability
- Education access and quality
- Healthcare access and quality
- Neighborhoods and safe housing
- Social and community context
In 2025, the Blue & You Foundation awarded $1,333,272 to nine organizations working to address the social determinants of health disparities in the state of Arkansas:
- American Heart Association – $111,200
- Arkansas Emergency Medical Foundation – $42,866
- Heartland Forward – $172,076
- Hispanic Community Services – $125,000
- Lyon College – $250,000
- Margie’s Haven House – $118,441
- Refuge Village – $210,000
- Ronald McDonald House Charities of Arkansas – $125,789
- Serve Northwest Arkansas – $177,900

Deadlines for Social Deteminants of Health Grant Cycle
- Letters of Intent will open on January 1 and close on January 12.
- Foundation staff will review letters of intent, and applicants will be notified of approval or decline by email on January 30.
- Full applications will open for approved letters of intent on February 2, and organizations will have until midnight on February 20 to submit their finalized applications.
- Applicants will be notified of final decisions by email on May 6.
