Social Determinants of Health Grants, $25,000 – $250,000
The Social Determinants of Health grants focus on the factors that impact health outcomes. They are the conditions in which people are born, grow, live, and work and the systems shaping the conditions of people’s daily lives. Grants will be awarded that address the following categories:
- Economic stability
- Education access and quality
- Healthcare access and quality
- Neighborhoods and safe housing
- Social and community context
In 2024, the Blue & You Foundation awarded $1,036,875 to 14 organizations working to address the social determinants of health disparities in the state of Arkansas:
- Arkansas Coalition for Obesity Prevention – $192,600
- City Connections – $35,000
- Conway Regional Health Foundation – $24,816
- EAST Initiative – $30,000
- Goodwill Arkansas Education Initiative – $50,000
- Grace Presbyterian Church – $13,000
- Hispanic Community Services – $118,795
- Kind at Heart Ministries – $10,000
- Little Rock School District – $13,710
- Northwest Arkansas Foodbank – $25,000
- Oasis of Northwest Arkansas – $9,500
- Restore Hope – $200,000
- University District Development Corporation – $200,000
- Wolfe Street Foundation – $114,454

Deadlines for SDoH Grant Cycle
Letters of Intent will open on January 1 and close on January 12.
Applicants will be notified by email on January 24, if the letter of intent is approved and the application process is open to the organization. The application period will close on February 14.
Award notifications will be emailed on May 6.
